The geographical division between the two virus groups has so far been concentrated in Cameroon - the only country where both groups have been detected.

Most cases have since been reported in rural areas of the Congo Basin rainforests and West Africa, and human cases of infection are increasingly being reported across Central and West Africa.Since 1970, human cases of monkeypox have been reported in 11 African countries: Benin, Cameroon, Central African Republic, Democratic Republic of the Congo, Gabon, Côte d'Ivoire, Liberia, Nigeria, Republic of the Congo, Sierra Leone and South Sudan. The true burden of monkeypox is not yet clear. For example, in 1996-1997, an outbreak was reported in the Democratic Republic of the Congo with a lower case fatality rate and an average.

The main prevention strategy for monkeypox is to raise awareness of risk factors and educate people about measures they can take to reduce exposure to the virus. Scientific studies are being conducted to evaluate the feasibility and appropriateness of vaccination for the prevention and control of monkeypox. Some countries have, or are currently establishing, policies for administering the vaccine to people at risk such as laboratory workers, rapid response teams and health workers.
