I create surveys using Google Forms because it is very flexible. It has a wide variety of possible questions and does not have a limit to the number of surveys you can create. It also provides an embed code so the form can be embedded into a website. This is great for small surveys. If your survey is more than one page Google provides a link you can add to your website that opens the form in a new page. The results of the survey can be emailed to any email address so you know what answers each survey taker gave. Lastly, there are several different styles of forms to choose from so that the form matches your website. I have taken a survey design course in college. I know how to use neutral words and order the questions so that the survey does not hint at choosing specific answers. I also know the basics of reliability and validity.
