Identify areas to improve the quality of clinical care to drive better patient care practices and drive down costs associated with practice transformation efforts. Guide physicians in the transition from fee-for-service to a value-based model which leverages technology and patient relationship management to achieve long-term gains in quality of care and population health management. Assist with writing Transforming Clinical Practice Transformation performance assessments, PCMH or any other NCQA applications, CMS grants, SAC applications and UDS reporting for FQHCs, MIPS and ACO reporting.
