Fluorosis mitigation program.

Fluorosis is a serious public health problem in many parts of the world, which can be corrected by proper education and intervention. Excess consumption of fluorine particularly through drinking water is the main causative reason for the development of fluorosis.This can be dental fluorosis, skeletal fluorosis or nonskeletal manifestations. Dental fluorosis: Among fluorosis only dental fluorosis is externally visible It is more prominant among children in the age group of 10- 15 years..In the initial stage the dental enamel shows chalky white patches, which may change the colour to yellow and brown and finally black. Pitting of teeth may happen in this situation.Apart from the physical condition the children will be reluctant to smile and may turn introvert. Skeletal fluororsis: In skeletal fluorosis, deformities and abnormal growth of skeleton is noticed.Nerves will be pressed by the skeleton and a lot of neurological problem will be developed. Nonskeletal manifestations: Certain internal organs will be affected in fluorosis resulting in related health problems.Even mail infertility may occur due to the deformities happen to the sperm. Education, Research and Public health intervention:Though consumption of water with high fluoride content is the main causative factor for the development of fluorosis, low consumption of calcium rich food and vitamin C coupled with the fact of fluoridated tooth paste in fluorosis endemic areas will aggravate the situation. Hence education to the Community, Health personnels,Policy makers, Planners and Executives is very important in controlling and preventing fluorosis in many parts of the world. Public health research has to be undertaken to find out the magnitude of the problem in various places .Intervention in terms of supply of drinking water with optimum fluoride content is necessary to control fluorosis. Fluoride is a double edged sward: Excess fluoride consumption will lead to fluorosis and low consumption will lead to dental caries.The optimum fluoride content in drinking water is considered as 1 ppm (parts per million). So 1 mg of fluoride in 1 lit of water is considered optimum. But in tropical countries where water consumption is more compared to that in temperate regions the fluoride content in water should be less than 1ppm.
